Registration Information
Limited, on-site registration will be available until 10 am on June 1st. Both cash or check will be accepted for on-site registration, however you run the risk of being turned away in the event we reach maximum capacity and we may not be able to offer you a box lunch in case we underestimate our final attendance figure and run out.
To register, please complete and print out this registration form (time saving tip: you can type your information directly into this pdf file before printing it; works best with Adobe Reader 7 or newer). Checks and purchase orders should be made out to Applied Science Associates. Save time on Friday by completing and printing out a registration form now to take with you to the conference.
If you are paying with a purchase order, Applied Science Associate's W-9 form is available on request from Greg Bonynge.
Sorry, no online registration is available, nor credit cards will be accepted for payment of the registration fee. Your registration fee is non-refundable, however you may transfer your registration once if needed.
